The Colorado Department of Public Health and Environment (CDPHE) is partnering with incentaHEALTH to offer incentaHEALTH’s online diabetes prevention program, #PreventDiabetes to Coloradans. This partnership has been made possible by grant funding and includes additional partnerships with employers and healthcare providers.
CDPHE serves Coloradans by providing public health and environmental protection services that promote healthy people in healthy places. The partnership seeks to increase access to a Centers For Disease Control (CDC) fully recognized National Diabetes Prevention Program (NDPP) across Colorado. incentaHEALTH plans to partner with community organizations to achieve the aim of increasing accessibility through a distance-based NDPP.
The incentaHEALTH #PreventDiabetes program is a year-long virtual lifestyle change program built upon incentaHEALTH’s evidence-based weight management platform. Individuals can reduce their risk for developing Type 2 diabetes with the support of app-based coaching with exercise plans and meal ideas, educational on-demand video sessions, the support of a certified Diabetes Lifestyle Coach and more. Progress is tracked through healthy selfie photographed weigh-ins with authenticated weight and Body Mass Index (BMI) data. Members are rewarded for achieving weight loss benchmarks every 90 days.
The outcomes for #PreventDiabetes members meet and exceed the CDC benchmark standards. The majority of members achieve at least the required 5% weight loss threshold. This milestone is considered the standard for preventing and reducing risk for developing Type 2 diabetes. incentaHEALTH’s success with these outcomes and cost savings can also be noted in Preventive Medicine study.At incentaHEALTH we help individuals, employers and health plans with weight loss and diabetes prevention solutions with our scalable, evidence-based digital platform.
